Midway University Athletics Participation Requirements
There are 4 documents that must be completed to participate in athletic activity at Midway University.
#1 Midway Pre-Participation Forms
#2 Midway Student-Athlete Medical History
#3 Front and Back of Insurance Card
#4 Midway Physical Form **Date must be AFTER June 1st, 2024**
#1 Midway Pre-Participation Forms, and #2 Midway Student-Athlete Medical History are fillable forms that can be completed on the website or the app.
#3 Front and Back of Insurance Card, and #4 Midway Physical Form will need to be uploaded.

- Accessing Healthy Roster:
- Getting an Access Code
- Access codes are sent out to the Midway email upon completion of tuition deposit for new students.
- If a new student has not gotten an invitation from Healthy Roster after checking the spam/junk box they can reach out to AT@midway.edu.
- Returning students will use the same Healthy Roster account and access as last year.
- If you do not recall your password please go through the ‘forgot password’ process.

- Finding the Forms
- On a PC:
- Once you have access to Healthy Roster you can access the forms by clicking on your name to view your profile.
- Once you open your profile you will see a tab that says “documents,” click that tab to pull up your documents section.
- In the documents section you will see a list of forms that are labelled missing.
- By clicking on a form, you can access it if it is fillable or be able to attach a file if it requires you to upload a file.
- You can also access a list of forms by finding the ‘Add’ button in the top right-hand corner.
- This will bring up a drop-down menu of document types that you can select, and then add attachments as needed.

- The phone app:
- Once you have access to Healthy Roster you can access the forms by clicking on your name to view your profile.

- By clicking on a form, you can access it if it is fillable, or it will allow you to scan a form directly with your phone’s camera.
